Death claim

- Insurance policy - Death claim request form (according to the Company’s form)
- Copy of death certificate
- Copy of identification card and copy house registration of the insured and every beneficiary
- Power of attorney and consent for medical history disclosure (according to the Company’s form)
- Declaration of attending physician (according to the Company’s form)
- Copy of daily police report (in case of unnatural death)
- Copy of autopsy report (in case of unnatural death)
Not over 90 days after the date that the Company received complete and correct documents.
Accident or health claim - Compensation request form in case of injury/illness (according to the Company’s form)
- Original medical expense receipt in case of medical expense claim request under PA or Hospital & Surgical (H&S) contract, or copy of medical expense receipt in case of claim request under Hospital Benefit (HB) contract
- X-ray result interpretation in case of broken bone
Not over 15 days after the date that the Company received complete and correct documents.
